BlephEx and OptiVize: Lid Margin and Meibomian Gland Cleaning

A two-step lid hygiene system from Meibum Scientific: BlephEx clears the lid margin and lash line of biofilm and debris; OptiVize targets the deeper biofilm inside the meibomian gland ducts themselves.

Key points

  • Available on the Dry Eye Rescue wholesale platform through Meibum Scientific.
  • BlephEx clears the lid margin and lash line of biofilm and debris.
  • OptiVize targets biofilm inside the meibomian glands, the deeper step.
  • A two-step lid hygiene system from Meibum Scientific.

BlephEx, Controlled Lid Margin Debridement

BlephEx is a precision medical device that uses a rotating micro-sponge tip to remove biofilm, scurf, and inflammatory debris from the lid margin and lash base. The procedure is performed by the clinician in-office and takes approximately 6 to 8 minutes for both eyes. It is typically performed before any thermal pulsation or IPL treatment to clear the lid margin access points, and can dramatically improve the effectiveness of downstream procedures.

Clinical application of BlephEx

  • Removes the biofilm layer that accumulates on the lid margin and contributes to chronic inflammation and obstructed meibomian gland orifices
  • Significantly reduces Demodex load on the lid margin when performed regularly
  • Improves accessibility to meibomian gland orifices before thermal expression
  • Can be repeated on a maintenance schedule, typically every 4 to 6 months

Clinical evidence for lid margin debridement

  • Korb & Blackie (2013), Cornea, Debridement-expression of the meibomian glands (removing lid margin debris before expression) resulted in significantly more glands yielding secretions compared to non-debridement expression. PubMed
  • TFOS DEWS III (2025), Blepharoexfoliation (lid margin debridement) supported as part of the lid hygiene and anti-Demodex treatment component of MGD management. American Journal of Ophthalmology

OptiVize, Intraductal Meibomian Gland Biofilm Removal

OptiVize takes the cleaning deeper, targeting biofilm that has formed inside the meibomian gland ducts themselves, not just on the surface lid margin. The device uses a vibrating tip introduced at the gland orifice to break down intraductal obstructions. It is used after BlephEx lid margin debridement as a deeper clearance step.

Clinical application of OptiVize

  • Indicated when lid margin debridement alone does not resolve gland obstruction
  • Particularly useful for patients with long-standing MGD and inspissated intraductal debris
  • Pairs with thermal pulsation, warming the meibum first, then OptiVize to clear the duct, then expression
  • Part of a comprehensive lid hygiene protocol before IPL or thermal pulsation procedures
