Indications and Usage
A carbonic anhydrase inhibitor indicated as adjunctive treatment of chronic simple (open-angle) glaucoma, secondary glaucoma, and preoperatively in acute angle-closure glaucoma to lower intraocular pressure. Also indicated for edema due to congestive heart failure and drug-induced edema, for centrencephalic epilepsies, and for prevention or amelioration of symptoms of acute mountain sickness.
Dosage and Administration
For glaucoma, 250 mg to 1 g per 24 hours, usually in divided doses for amounts above 250 mg. For epilepsy, 8 to 30 mg/kg daily in divided doses; for edema, 250 to 375 mg once daily. For acute mountain sickness, 500 mg to 1000 mg daily in divided doses, ideally started 24 to 48 hours before ascent and continued during ascent.
Warnings and Precautions
- Fatal sulfonamide-type reactions have occurred rarely, including Stevens-Johnson syndrome, toxic epidermal necrolysis, hepatic necrosis, and blood dyscrasias; discontinue at first sign of rash or serious reaction
- Concomitant high-dose aspirin may cause anorexia, tachypnea, lethargy, coma, and death
- May cause metabolic acidosis and electrolyte imbalance; monitor blood counts and electrolytes during therapy
- Use caution in patients with impaired respiratory function, diabetes, or gout, and avoid in severe renal or hepatic impairment
Contraindications
Contraindicated in marked kidney or liver disease or dysfunction, depressed sodium or potassium levels, adrenocortical insufficiency, hyperchloremic acidosis, cirrhosis, hypersensitivity to sulfonamides, and long-term use in chronic noncongestive angle-closure glaucoma.
Adverse Reactions
Common adverse reactions include paresthesias, hearing disturbances or tinnitus, loss of appetite, taste alteration, nausea, vomiting, diarrhea, polyuria, and drowsiness or confusion. Metabolic acidosis, electrolyte imbalance, photosensitivity, and blood dyscrasias may also occur.
Storage and Handling
Store at 20°C to 25°C (68°F to 77°F), with excursions permitted to 15°C to 30°C (59°F to 86°F). Dispense in a tight, light-resistant container.
