Indications and Usage

Production of local or regional anesthesia and analgesia for surgical, dental, diagnostic, therapeutic, and obstetrical procedures.

Dosage and Administration

Dose depends on the procedure, tissue vascularity, and patient factors; the maximum single dose is generally 4.5 mg/kg, not usually exceeding 300 mg. For epidural use, give incrementally in 3-5 mL doses, aspirating before each injection to avoid intravascular or intrathecal placement; reduce doses for pediatric and debilitated patients.

Warnings and Precautions

  • Dose-related systemic toxicity requiring cardiovascular and respiratory monitoring
  • Risk of methemoglobinemia
  • Severe toxicity from unintended intravascular or intrathecal injection; aspirate before injecting
  • Chondrolysis reported with intra-articular infusion (an unapproved use)

Contraindications

Known hypersensitivity to lidocaine, other amide-type local anesthetics, or any component of the formulation.

Adverse Reactions

CNS effects such as lightheadedness, nervousness, dizziness, drowsiness, and tinnitus, and cardiovascular effects including bradycardia, hypotension, and cardiovascular collapse; allergic reactions and methemoglobinemia may also occur.
