Indications and Usage

Production of local or regional anesthesia by infiltration and peripheral nerve block techniques, including retrobulbar block, and topical anesthesia of the mucous membranes of the respiratory tract.

Dosage and Administration

For a 70 kg adult, retrobulbar injection typically uses 3-5 mL (120-200 mg) and transtracheal use 2-3 mL. Total dose should not exceed approximately 4.5 mg/kg (up to 300 mg); reduce for pediatric patients.

Warnings and Precautions

  • Should be used only by clinicians trained in managing local anesthetic toxicity
  • Risk of methemoglobinemia
  • Aspirate before injection to avoid intravascular administration
  • Chondrolysis reported with intra-articular infusion (an unapproved use)

Contraindications

Known hypersensitivity to amide-type local anesthetics.

Adverse Reactions

CNS effects including tremor, dizziness, and seizures with respiratory depression, and cardiovascular depression such as bradycardia, hypotension, and cardiovascular collapse; allergic reactions are rare with amide anesthetics.

Storage and Handling

Store at 20°C to 25°C (68°F to 77°F), USP controlled room temperature; discard unused portion (single-dose, preservative-free).
